PROJECT FAITH WAIVERS
SPIRIT WEEK INFORMATION

Complete the Project Faith Waivers - Just two quick steps before we serve together!

Participation in Project Faith requires two completed waivers for every participant—one for Faith Christian School and one for the organization we are serving. These forms are mandatory and must be submitted before students and adults can participate in any Project

Faith activities, including transportation, service projects, or event-day programming. To ensure we have the necessary permissions and safety information, please complete the waivers by the deadline below. Volunteers cannot be permitted to participate without these signed waivers.

Download, print, and submit the completed form to Margo Vitello.
